*NOW CLOSED* Dementia survey

Are you living with dementia, or caring for someone who has dementia?

Primary Health Tasmania is working with Primary Health Networks in Victoria to understand:

• your experiences with your local doctor (GP)
• what information and health and support services you have found helpful or you have needed, but not been able to find.

We will use this information to help GPs improve the way they support people with dementia (and their carers), and to help us understand what information resources may need to be developed.

Please complete our 20-minute survey. At the end of the survey, you can enter a draw to win a $200 EFTPOS card.
