Meet the Care@home team to hear about the expanded Care@home service. Care@home is a statewide interdisciplinary service that remotely supports patients, to better self-manage their illnesses in their own home using virtual care technologies. Care@home is now able to take on other conditions in the acute and chronic space, promoting integration across primary and secondary care to enhance patient health outcomes. Additionally, our GP Now service provides crisis support and intervention to communities where private general practice has experienced challenges.

In this presentation information will be provided on the following service pathways which includes.

  • Acute Virtual Monitoring Program
  • Chronic Disease Management Program
  • GP Now
