This session, facilitated by Primary Health Tasmania in partnership with Kidney Health Australia, will provide an overview of the early detection and management of chronic kidney disease (CKD) in the primary care setting. It will highlight the strong links between CKD, diabetes and cardiovascular disease, and outline practical approaches to identifying at-risk patients, initiating appropriate investigations, and implementing evidence-based management strategies to slow or prevent disease progression. The session will also emphasise the importance of integrated care in reducing morbidity and improving patient outcomes.
