Healthy Hearts for Tasmania is a collaborative project between the Department of Health, Heart Foundation and the University of Tasmania. This interactive session will involve the Healthy Hearts project team giving a briefing on the results of the study so far, including the issues and opportunities identified relating to cardiac rehabilitation in Tasmania. Then you will have an opportunity to offer your perspectives on the challenges and opportunities for cardiac rehabilitation in Tasmania from the perspective of primary care. These include the interface between primary care and subacute hospital services in relation to cardiac services, content and delivery of cardiac rehabilitation including digital and community options, and how to provide information about cardiac rehabilitation for health professionals and patients.

Participants will be reimbursed for their time. This has been accredited for 1 hour of Review Performance, ACCRM CPD pending.

Learning Outcomes:

  • Understand the evidence for Cardiac Rehab improving outcomes for people with CVD.
  • Understand the core components of cardiac rehabilitation based on Australian Cardiovascular Health and Rehabilitation Association (ACRA) guidelines.
  • Understand the key challenges and opportunities relating to creating a statewide evidence-based cardiac rehabilitation model of care.
  • Contribute to discussion regarding a new model of care for cardiac rehabilitation in Tasmania.

Speaker bio:

Professor Seana Gall, a cardiovascular epidemiologist, and leader of the Cardiovascular Research Flagship, and Cardiovascular Health research theme, at the Menzies Institute for Medical Research, University of Tasmania.

Associate Professor Pieter Van Dam, who is based in the School of Nursing at the University of Tasmania, with a focus on health service improvement, evaluation and leadership development.

Seamus Barker, a postdoctoral research fellow at the Menzies Institute for Medical Research, with a focus on implementation, policy, and qualitative research.
