Healthy Hearts for Tasmania is a collaborative project between the Department of Health, Heart Foundation and the University of Tasmania. Phase 1 of the project was completed in 2023 and included an environmental scan and gap analysis of cardiac rehabilitation and secondary prevention services in Tasmania. Phase 2 of the Healthy Hearts project is now underway and involves developing a model of care for cardiac rehabilitation in Tasmania, including localised service models. A key part of Phase 2 is consultation with people with lived experience of cardiac illness and GPs to ensure the model of care meets the needs of the community.

Participants will be reimbursed for their time. RACGP and ACCRM CPD pending.
