My Health Record Benefits for Residential Aged Care Teams

MyMedicare is a new voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner (GP) and primary care teams. Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes.

The General Practice in Aged Care Incentive (GPACI) will make it easier for older people living in residential aged care homes to receive regular visits and care planning services from their responsible GP and practice.
