TRACER is the Tasmanian Residential Aged Care Enhanced Response pathway to connect 000 calls triaged by Ambulance Tasmania (AT) as non-urgent, from Residential Aged Care Homes directly to the AT Clinical Hub for virtual clinical assessment.
Where appropriate, referral will be made to enable care to be provided in Residential Aged Care Homes and avoid unnecessary transport to the Emergency Department or re-triaged in priority for ambulance response reflecting the resident’s needs.
Representatives from the Ambulance Tasmania TRACER project team will be providing information on TRACER, and how it will support care in Tasmanian Residential Aged Care Homes.
