Primary Health Tasmania has a role in supporting local implementation of the Australian Government’s MyMedicare voluntary patient registration model.
MyMedicare aims to formalise the relationship between patients, their general practice, GP and primary care teams. General practices can register as a MyMedicare practice, and then patients can register a MyMedicare general practice as their regular practice.
When a general practice and its patients register in MyMedicare, the Australian Government will provide extra funding to help the practice provide targeted care based on the needs of its patients.

General Practice in Aged Care Incentive
MyMedicare general practices can register for the General Practice in Aged Care Incentive, which aims to increase access to GPs for people living in aged care homes.
The incentive program supports GPs to provide regular visits and care planning services to their regular patients in homes. The aim is to reduce avoidable hospitalisations and support the continuity of care for residents.
