The Guidelines for Shared Transfer of Care aims to help health and community organisations initiate and sustain strategies to improve shared transfers of care.
The guidelines include the five Sharing Points for quality shared transfer of care, and a strategy for action to support the implementation of the Sharing Points.
They also include a number of tools for organisations to trial and/or adapt, and additional capacity-building resources.
